Mechanism of Action
Anti-inflammatory agent that inhibits microtubule polymerization, impairing NLRP3 inflammasome assembly and IL-1β secretion — a key inflammatory pathway in atherosclerotic plaque progression. FDA-approved (Lodoco) for cardiovascular risk reduction in established CAD. Addresses "residual inflammatory risk" not covered by lipid-lowering therapy.
Dosing Notes
0.5 mg once daily (Lodoco formulation). Approved June 2023 for adults with established atherosclerotic disease or multiple cardiovascular risk factors.

LoDoCo2 trial (2020): colchicine 0.5 mg/day in patients with chronic coronary disease reduced composite cardiovascular event rate by 31% vs placebo (HR 0.69, 95% CI 0.57–0.83). COLCOT trial (2019): colchicine after MI reduced risk of ischemic CV events by 23% vs placebo. FDA approved Lodoco for this indication in June 2023.
